Contacting and Scheduling an Appointment with Dr. DiMaio

  1. Contact the scheduling coordinator
  2. You will be sent a patient packet
  3. You will be asked to supply:
    • Medical History (past surgeries, illnesses, etc.)
    • List of allergies
    • Immunizations
    • Records, reports
    • If referred, please provide referral name, number, and reason
  4. Fill out the forms in your patient packet:
    • Confidential Health Questionnaire
    • Social History
    • Medical History
    • Surgical History

  5. Send or bring your information, CDs, and scans to the office.

**Note: DO NOT MAIL XRAYS TO THE OFFICE. We cannot return them to you.

Your first visit with Dr. DiMaio

  1. Arrive at the clinic location 15 minutes before your appointment
  2. Expect to bring the following with you to your first clinic visit:
    • Completed patient packet forms
    • Medical History Records
    • List of allergies
    • XRAYS  * Do not mail xrays to the office. We cannot return them to you*
    • Referral documentation
  3. Filled out patient packet forms.
  4. Prepare your questions for Dr. DiMaio:
    What kind of questions should I ask? 

    • Am I a candidate for this particular operation?
    • Typically, how long does this procedure take?
    • What can I bring with me to the hospital?
    • How long will I be in the ICU?
    • When can I continue regular activity after surgery?

General Instructions in Preparing for Surgery

  1. Do not eat 24 hours before surgery. Typically, you are allowed small amounts of clear fluids (no soda, no juice).
  2. Wear comfortable loose clothing that you will be able to change into after surgery.
  3. Remove ALL jewelry.
  4. Items you are allowed to bring with you to the hospital:
    Toiletries: toothpaste, toothbrush, deodorant, lipbalm, shampoo etc.
    Slippers
    Books, portable dvd players etc.
  5. Go to the surgical procedures section if you are interested in learning a little
    more about your surgery.

Post-Procedural Care and Follow-up

  1. Be clear on instructions from Dr. DiMaio on how to treat and medicate your incisions in after-surgery care.
  2. Ask your doctor when you may resume normal activity such as (lifting, exercising, working, etc.)
  3. Follow directions carefully on prescribed medication.
  4. Call to set up your follow up appointment with Dr. DiMaio.
  5. Follow instructions from the coordinator to have the appropriate x-rays and scanning done.
  6. When arriving at the clinic, remember to bring:
    • Any questions that you may have for Dr. DiMaio
    • Post-surgery scans and xrays
    • Paperwork and documentation issued to you from surgery
